We are inviting families of children and young people aged 0–25 with Special Educational Needs and Disabilities (SEND) to take part in our Annual Survey 2026.

At Leeds Parent Carer Forum, we work with Leeds City Council, schools, health services and social care to make sure families’ voices are heard. Your experiences help us challenge, influence and improve services across Leeds.
We know things can be difficult right now, and many families are facing challenges. That’s why it’s so important that we hear from you, whether your experience has been positive, challenging, or somewhere in between.
Why your feedback matters
Your responses help us to:
- Identify what is working well
- Highlight where things need to improve
- Share real experiences with services and decision-makers
- Shape our priorities for the year ahead
- Track changes over time
Every response helps build a clearer picture of what families are experiencing and where support is needed most.
What the survey covers
The survey is split into short sections and looks at your experience across education, health and social care.
This includes:
- Your child or young person’s needs and support
- Education and EHCPs, where relevant
- Experiences of services such as health, mental health, social care and short breaks
- Communication from services and awareness of the Leeds Local Offer
- Preparation for adulthood for young people aged 14+
A focus on health
This year, we are asking more detailed questions about health. This includes access to services, neurodevelopmental support such as autism and ADHD, mental health, feeding, sleep, and hospital care.
Your feedback will help us in our work with NHS partners to improve support for families.
We also include an open question so you can tell us about anything we may have missed. This is one of the most important parts of the survey.
Taking part
- The survey takes around 20 to 30 minutes to complete
- You can skip any questions that are not relevant
- You can complete it on a phone, tablet or computer
To help us get the most accurate picture, please complete one survey per child or young person.
Prize draw
As a thank you, two parent carers who complete the survey will be selected at random to receive a £30 Amazon voucher. Entering the prize draw is optional.
