The Leeds Social Care Transition Team will be joining us to talk about the support available for young people aged 14–25 with diagnosed disabilities and/or complex health needs. This session is aimed at parents and carers who would like to understand more about preparing for adulthood and the transition into Adult Social Care.

- Who the Transitions Team are;
- Criteria for support from the Transitions Team;
- Alternative pathways to Adult Social Care (ASC);
- How the team works with young people under 18 and over 18;
- Accommodation options;
- Moving on from the Transitions Team; and
- Resources and questions.
The team works with young people who already have an existing package of care in place, such as Direct Payments or Respite support, helping them build independence and prepare for adult life.
This will be an opportunity to hear directly from professionals, ask questions, and learn more about support available in Leeds.
