Leeds City Council has launched a new online form to help parents and carers track the progress of their child’s Education, Health and Care Needs Assessment (EHCNA), in response to feedback from families.
To use the form, you’ll need:
* Your child’s Synergy number
* Their full name and date of birth
* Your email address (must match the one on record)�
